In addition to completing our online application, authorizing TEDC to review personal information, and certifying the accuracy of all information provided, you MUST email the following documents to our secure email address loans@TEDCnet.com(Please include the name of your business on the SUBJECT line of your email message.)

  • Payroll report on employees who were paid salaries, wages and benefits as of February 1, 2020
  • 2018 or 2019 Personal Federal Tax Return for each owner
  • 2018 Business Federal Tax Return
  • 2019 Business Federal Tax Return or Year End 2019 Balance Sheet and Profit & Loss Statement
  • 2020 Year to Date Balance Sheet and Profit & Loss Statement (or business checking account statement January, February & March 2020)
  • Personal Financial Statement (see Form)

  • SBA Form 912: Statement of Personal History (See Form)

  • Debt Schedule (see Form)

  • Personal Resume for each owner/guarantor
  • For Start-ups: Detailed Business Plan with 3 years of financial projections (1st year detailed by month)
  • Personal Federal Tax Returns of each owner/guarantor for 3 years
  • Business Balance Sheet and Income Statement dated within 60 days of application
  • Business Balance Sheet and Income Statement for last 3 years
  • Business Federal Tax Returns for last 3 years
  • $35 Credit Report/Application Fee for each 20% owner or guarantor
  • Personal Financial Statement (included in application packet)

  • SBA Form 912: Statement of Personal History (included in application packet)

  • Employee Survey Form - Standard (See Form)

  • Employee Survey Form - Qualified Census Tracts (See Form)
